|  | |  | | Patient Number 1: Liposuction | | Actual Patient -- Results vary from patient to patient |  | | Notes: This is a 26 year old female from the Scottsdale, Arizona area who came to my Paradise Valley, Arizona area medical practice with concerns about her breasts and body. The patient reported that her breasts were too large and caused problems such as back and shoulder pain, rashes, and difficulty finding clothes that fit. She was also unhappy with the excess fat of her abdomen, back and flank (love-handle) areas.
I felt that this patient would be an excellent candidate for breast reduction surgery. This would both lift, and reduce the weight of her breasts, which in turn would make her breasts more attractive and more comfortable for her.
With regards to her body, I felt that liposuction would be a reasonable start, given that she had good quality, elastic skin. I explained that she would need to start a diet and exercise program after she healed to maintain her results (she was unable to exercise effectively before surgery due to her breast discomfort).
During surgery I performed a breast lift / reduction on both sides. This included reshaping the breast tissue, and internally releasing the upper breast tightness to allow for a more rounded breast. The incision pattern I used was the Wise pattern (anchor scar) in order to achieve maximum tightening and lift for this particular patient.
For the liposuction, I used Power-assisted liposuction, and removed 3300cc (total) of aspirate from the abdomen, back and flanks. I also removed 170cc from each bra fat area to help further shape the breasts.
This patient did well after surgery, and noted an immediate resolution of her breast discomfort symptoms. Once she healed fully, she began to follow a serious diet and exercise program. Between the surgery and the diet and exercise, this patient has lost over 30 pounds. She continues to lose weight and is extremely happy with her new body image.

|  | |  | | Patient Number 2: Liposuction | | Actual Patient -- Results vary from patient to patient |  | | Notes: This is a 28 year old Phoenix area female who came to my Scottsdale area medical office with complaints of excess fat of the abdomen, back, flanks, hips and inner / outer thighs. Despite significant exercise and diet, she had difficulty gaining the contours she desired.
Given this patient's healthy, elastic skin and focal fat collections, I felt she would be a good candidate for power assisted liposuction of the above mentioned areas. The goal was to create a more natural, streamlined appearance to her torso and legs and, understanding that she would continue to exercise and maintain a healthy diet after recovery from surgery.
Power assisted liposuction was performed, and excellent contouring results were achieved. This patient had approximately 4 liters of aspirate removed overall. She is very happy with her results, and reports fitting much better in her clothes. She has also gained a renewed sense of self-confidence.

|  | |  | | Patient Number 3: Liposuction | | Actual Patient -- Results vary from patient to patient |  | | Notes: This is a 23 year old mother from Sedona, Arizona who presented to my Scottsdale, Arizona office with complaints regarding her breast shape, size and asymmetry. She was also unhappy with the appearance of her abdomen and torso. She had never liked the appearance of her breasts, and this was her primary concern, but she was also interested in sculpting her torso as well.
Due to the significant issues with her breast shape (called tuberous breasts),and the excessive spacing of her cleavage and enlarged areolas, I advised that she would need both a breast enhancement (breast augmentation) as well as a breast lift (mastopexy). The breast lift I recommended in her case is called a periareolar mastopexy, which means that all of the tightening is done around the areola to avoid any scars on the breast skin itself. I also knew that she would need different sized implants to achieve good symmetry.
Regarding her abdomen, I felt that liposuction (liposculpting) of her central abdomen and flanks(back and torso) would enhance and balance her overall appearance. I performed a breast augmentation using a 350cc implant on the right and a 300cc implant on the left. The implants were Mentor smooth, round, moderate profile saline breast implants. They were placed under the pectoralis muscle on both sides. After this, tightening of the breasts was performed with a "periareolar mastopexy".
After I finished the breast surgery, I performed "Power-Assisted Liposuction", using a special device that vibrates the suction tube, allowing it to pass through tissues with less trauma, bruising and pain (in my experience). Careful sculpting of the torso was performed, and a total of 2.3 liters of aspirate (fat and fluid) was removed. As you can see, this procedure completely transformed her body. As a side benefit, I was able to convince this patient to quit smoking for the surgery, and she has since quit altogether. Additionally, she was inspired by her surgery to improve her diet and exercise habits, and continued to slim down after surgery.

|  | |  | | Patient Number 4: Liposuction | | Actual Patient -- Results vary from patient to patient |  | | Notes: This patient is a 42 year old female from the Phoenix, Arizona area who presented to my Scottsdale, Arizona area medical practice with complaints about excess fat of the inner and outer thighs. This patient is very involved in athletics, and explained that no matter how much exercise she did, she could not significantly change the shape of her thighs.
I explained to the patient that many people have areas of fat that are disproportionate and are resistant to diet and exercise. Often, people have a genetic predisposition to hold on to fat in certain areas of their bodies. I recommended power assisted liposuction of the inner and outer thighs to improve her leg contours and allow her to fit better into jeans and other types of pants and shorts.
I performed power assisted liposuction and removed large amouts of pure fat from the inner and outer thigh of each leg. The patient noticed an early, dramatic improvement and was very happy with her new appearance. At the time of these photos, the patient is only 6 weeks out from surgery. She can expect significant further improvement over the next 6-8 months.

This is a 33 year old female from the Scottsdale area who came to my office with concerns about the appearance of her abdomen, flanks and back. The patient had previously had an abdominoplasty by another surgeon, and was unhappy with the overall result. Specifically, she had excess skin remaining on her sides, and significant excess skin and fat in the pubic region. Furthermore, the patient was unhappy with some residual irregularities of the right upper abdomen.
With regards to her back and flanks, the patient disliked the "rolls" of excess skin and fat that caused creases in her skin. Finally, the patient wished to have an enlargement of her breasts with a lift, and removal of excess fat in the underarm/ bra areas.
I had a lengthy discussion with the patient regarding what could realistically be accomplished with surgery. Revision surgeries are usually more difficult than the original surgery due to preexisting scarring and other factors not present in a first-time surgery. Due to her overall body shape and size, the goal of surgery would be to smooth out her abdominal area, streamline the flanks and pubic area, and flatten out the rolls of skin and fat on her back. This was accomplished with a revision abdominoplasty (tummy tuck) focusing on the lower abdomen and combined with liposuction of the right upper abdomen, hips, flanks, pubic area and back. Between 150 and 400cc was suctioned from each different area.
Currently, the patient is scheduled for a second phase of surgery where I will perform a breast lift with an augmentation, combined with liposuction of her underarm/ bra areas. This will complete the transformation of this patient's torso. This patient was extremely happy with the improvements from the first phase of her surgery, and she is very excited to complete the remaining surgical procedures.
